HIPAA Compliance

The HIPAA (Health Insurance Portability & Accountability Act) is a federal law that establishes national standards to protect sensitive patient health information from being disclosed without a patient's consent.  It ensures the confidentiality, integrity, and availability of personal health data across the United States.  We take HIPAA compliance very seriously at Marshall Area Fire Fighters Ambulance.  Below you will find links to all of the important HIPAA documents provided for you.

Patient Request for
Access to Protected Health Information

This document may be filled out if you wish to request access to your PHI (Protected Heath Information).  This will guide you on how to obtain the information & who you can add to obtain a copy of your records.  

HIPAA Complaint Form

This form can be utilized if you feel that MAFFAA has not complied with the requirements related to Protected Heath Information (PHI) as described in our Notice of Privacy Practices.
